服務退出手續
機構會員
機構會員須在本港提供服務,並符合以下其中一類:
(i) 社會福利署/非政府社會福利服務機構
(ii) 香港教育學院、訓練社工及幼兒工作員的院校
(iii) 幼兒中心/幼稚園/中、小學校
(iv) 醫管局轄下的服務單位
(v) 其他政府機構
(vi) 非牟利臨床心理學及教育心理學服務組織
(vii) 已註冊的非牟利慈善/宗教服務團體
學生會員
就讀社會工作、幼兒教育及香港教育學院的學生
申請入會方法
(1) 符合以上學生會員資格的人士,須填妥一份學生會員申請表,連同
     有效學生証副本,郵寄或傳真(2572 2248)至本中心。


(2) 符合以上機構會員資格的機構,須填妥一份機構會員申請表,連同
     所需證明文件,郵寄或傳真 (2572 2248) 至本中心。


(3) 以上第(i)至(v)類機構會員及學生會員的申請,本中心一般會在兩星
     期內處理,並會儘快以書面通知有關申請的結果。


(4) 以上第(vi)及(vii)類機構會員的申請,審核程序包括:
      a.申請機構須接受本署的探訪,以了解所提供的服務。
      b.申請機構須提交文件以證明其非牟利的性質,認可文件如
         「根據稅務條例第88條認可慈善機構及信託團體名單」,
         申請機構可於稅務局網頁
         (http://www.ird.gov.hk/chi/tax/ach_search.htm)下載有關
         文件。
      c.申請機構所舉辦的活動須以推動家庭生活教育為目的。


本中心在取得以上資料後,會儘快以書面通知申請的結果。
服務退出手續
(1) 會員如欲退出服務,須填妥會員退會通知表格,並交回本中
     心。


(2) 在下列情況,本中心有權要求會員退出服務:
  • 會員屢次違反借用守則或遺失/損壞借用物品
  • 會員不再符合接受服務資格

    Withdrawal of Membership
    Agency Member
    Agency member is required to provide service in Hong Kong and belongs to one of the following groups:
    (i) SWD/ Non-Governmental Welfare Organisations
    (ii) HK Institute of Education, Institutes which provide social work
          or child care training
    (iii) Nurseries/ Kindergartens/ Secondary and Primary Schools
    (iv) Units under Hospital Authority
    (v) Other Government Organisations
    (vi) Non-profit-making Psychological/ Education Psychological
          Units
    (vii) Registered Non-profit-making Charitable/ Religious
          Organisations
    Student Member
    Students of HK Institute of Education and students studying social work or child care courses.
    Application Procedure
    (1) Student eligible to apply as member should complete and send
         the "Student Membership Application Form" with a copy of
         valid student card to FLERC by fax (2572 2248) or by post.


    (2) Agency eligible to apply as member should complete and send
         the "Agency Membership Application Form" with the
         required documents to FLERC by fax (2572 2248) or by
         post.


    (3) For the application of student member and agency member for
         catagories (i) to (v) above, FLERC will inform the result to the
         applicant by post in 2 weeks' time.


    (4) For the application of agency member for catagories (vi) and
         (vii) above, the applicant has to follow the below vetting
         criteria:
        a. accept SWD's visit to the agency
        b. produce the "List of Approved Charitable Institutions and
            Trusts of a Public Character" as proof of its charitable status
             Applicant may download the document from the Website of
             Inland Revenue Department
             ( http://www.ird.gov.hk/eng/tax/ach_search.htm)
        c. produce proof to indicate its programmes are relating to
            family life education


    After receiving the above information, FLERC will give a written reply to the applicant as early as possible.
    Withdrawal of Membership
    (1) For withdrawal, member is required to notify FLERC by
         completing the "Withdrawal of Membership" form.


    (2) Under the below circumstances, FLERC has the right to
         request member to withdraw membership:
  • the member repeatedly violates borrowing rules and procedures
        or damages resource materials.
  • the member no longer fulfils the criteria of membership.
